This is a demo / example of a status update form. The customer has already been entered into the system, and selected for a status update. The actual fields, and contents of dropdowns and checkboxes would be determined later, so for now we made some up as placeholders.

The customer "code" can be automatically generated as the customer information is pulled from the database, but for this demo it is just a blank field.
